I think that while a cardboard box can be used as a stealth item, I can't thik of any of Cap's antagonist's being dumb enough to fall for it.

No, that's not how it works at all. People don't see the box moving. They just turn around - oh, it's just a cardboard box. Nothing to worry about. Turn back. Turn around again - oh, it's just a cardboard box. And they still don't worry about it, even though it's moved a few yards closer to them.
So, they don't actually see the box moving, because it happens while they're not looking. Like Daniel Hammer's teleportation. _________________

I've never played Metal Gear, so I don't know if the game is programmed so nobody sees the box, but in a more realistic world, people would see it at some point or another.
